New Hampshire was awarded $204 million from the Centers for Medicare & Medicaid Services to implement the first budget period of its five-period Rural Health Transformation (RHT) Plan. The State’s RHT Plan is administered by the Governor’s Office of New Opportunities and Rural Transformational Health (GO-NORTH). GO-NORTH contracted with the Community Development Finance Authority (CDFA) to invest more than $40 million per year to create the GO-NORTH Rural Community Health Infrastructure Programs, which will provide resources to nonprofits and municipalities to support infrastructure improvements to health and community facilities across the State.
The GO-NORTH Rural Community Health Infrastructure Program is a competitive program that will invest funds to rehabilitate rural health infrastructure that are included in and aligned with the initiatives and outcomes committed to in New Hampshire’s GO-NORTH Plan. The program will also provide technical assistance to prospective applicants and awardees to help facilitate successful implementation, from grant application through project completion.

CDFA hosted a virtual webinar on June 11, 2026 for organizations interested in applying for the GO-NORTH Rural Community Health Infrastructure Program funds or learning more about the program. Topics covered include: an overview of the Program for Budget Period 1, eligible applicants and activities, and next steps in the application process.
The webinar recording and presentation are available for those who were unable to join live but are interested in learning more about the program. Grant writers, grant administrators, municipalities, nonprofits and any projects that intend to apply for the program are encouraged to review these materials.
REQUEST FOR ELIGIBILITY DETERMINATION FORM NOW AVAILABLE
Organizations interested in seeking GO-NORTH Rural Community Health Infrastructure Program resources can now complete the Request for Eligibility Determination form on CDFA’s Grant Management System (https://nhcdfagrants.org/).
The Request for Eligibility Determination is the first required step in the application process for the GO-NORTH Rural Community Health Infrastructure Program. It requires you to provide a concise, high-level overview of the proposed project. Please note this is not a full application.
Requests will be reviewed on a rolling basis by CDFA and GO-NORTH. Approval of a Request for Eligibility Determination does not guarantee funding but provides the opportunity for the eligible entity to submit a full application to be considered in a competitive funding round.
